Outcomes of stereotactic ablative body radiotherapy for treatment of oligometastatic gynecological cancers: a narrative review

Abstract
Stereotactic ablative body radiotherapy is an advanced radiotherapy technique well established in other tumor sites and increasingly utilized for oligometastatic gynecological cancer, with potential benefits for disease control and increased survival. Given the paucity of higher-level evidence, uncertainty still exists regarding future directions for the use of stereotactic ablative body radiotherapy. This review aims to summarize the published literature regarding local control, progression-free survival, and toxicities associated with stereotactic ablative body radiotherapy within this patient population, and to identify emerging areas of interest, including single-fraction stereotactic ablative body radiotherapy and concomitant systemic therapy. A comprehensive literature search was conducted using the OVID Medline, EMBASE, and PubMed databases to identify relevant studies. Inclusion criteria included clinical trials and retrospective studies reporting the outcomes of local control, progression-free survival, and toxicities in adult female patients with oligometastatic gynecological cancer. Despite significant heterogeneity in the patient populations, making comparison between studies difficult, the reported outcomes were promising. In total, 990 patients were involved in the 17 studies reviewed, with various gynecological primary malignancies and 1 to 5 extra-cranial metastases. Several studies reported local control rates >80% at 1 year. Median progression-free survival ranged from 7.8 to 32 months. Toxicities were minimal, with most studies reporting no (3) grade III toxicities, and only one grade V event was reported across all studies reviewed. Factors associated with improved outcomes included nodal metastases, smaller lesion size, and higher biologically effective dose. In conclusion, stereotactic ablative body radiotherapy appears to be an effective and safe treatment option for selected patients with oligometastatic gynecological cancer. Further research is indicated to determine long-term outcomes, optimize patient selection, and evaluate emerging trends such as concomitant systemic therapy and single-fraction stereotactic ablative body radiotherapy.
