Abstract
This chapter explores the evolving hospital-based models of care designed to support people living with Long COVID. It begins with an overview of key hospital services for acute COVID-19 infection, with a focus on interventions that reduce disease severity and enhance recovery, including access to early in-reach rehabilitation. Risk factors for Long COVID that may be modifiable and potential preventative strategies are highlighted, emphasising the importance of early interventions, vaccination and pharmacological treatments. The chapter concludes with a discussion on hospital-based multidisciplinary Long COVID clinics, underscoring their role in addressing the diverse and complex needs of people with this condition. Two Australian multidisciplinary clinic models are presented as case studies.
